Quotation marks; exceptions to the Guidelines

A difficult structure to format. This approach keeps the emphasized text together. Originally, it was done as: <i>they</i> '<i>defy criticism</i>.' because that conforms to Guidelines. After several experts reviewed it, the single markup made more sense, as did leaving a [**note] for the Post-Processor.
