Continued entries and sub-entries

This is a continuation of the previous example. It is the next page in the book from which both are taken.

The first word and its page number are not indented here because the formatter knew they continue the sub-entry that began on the previous page.

NOTE: it won't always be obvious whether something is a new sub-entry or a continuation. Although it would be best if you find out by looking at the previous page, it is acceptable to treat this as a new sub-entry by indenting it.
